Clinicians
know that it is critical to document medical necessity in
order to demonstrate skilled service. It is understanding
just what the intermediary expects to see in order to justify
medical necessity that often causes some confusion for clinicians.

Therapy
re-evaluations... What are they and when are they covered?
Many
practitioners are unsure of when to use the Therapy re-evaluation
codes (97002 PT re-evaluation and 97004 OT re-evaluation).
As a result, practitioners either opt not to use the codes
at all or potentially misuse them.
